Required Enrollment Documents

When you register your child, you will need the following:

• Birth Certificate or affidavit explaining the inability to provide a copy of the birth certificate. A.R.S. §15-828.
• Proof of immunizations (your child will not be admitted unless their immunizations are up-to-date).

• Name/Address of previous school.
• Proof of legal custody may also be required.
• Parent/Guardian picture ID.
• Proof of residence.
